Adult and Child Services:
-
Diagnostic Assessment
An intensive clinical and
functional face-to-face
evaluation of a recipient’s
mental health, developmental
disability, or substance abuse
condition that results in the
issuance of a
Diagnostic/Assessment report
with a recommendation regarding
services and provides the basis
for the development of a Person
Centered Plan (PCP). It also
evaluates the recipient’s level
of readiness and motivation to
engage in treatment. Results
from a Diagnostic/Assessment
include an interpretation of the
assessment information,
appropriate case formulation and
an order for immediate needs and
the development of a Person
Centered Plan.

Community Support
Consists of mental health and
substance abuse rehabilitation
services and supports necessary
to assist the client in
achieving and maintaining
rehabilitative, sobriety, and
recovery goals. These
individuals provide coordination
and oversight of initial and
ongoing assessment activities,
initial development and ongoing
revision of Person Centered
Plans, monitoring of
implementation of the plan, as
well as coordination of movement
across levels of care, directly
to the person and their family,
and coordinates discharge
planning and community re-entry
following hospitalization,
residential services and other
levels. Community Support
Workers provide skill building
including daily and community
living skills, socialization
skills, adaptation skills,
development of leisure time
interests/activities, symptom
management skills, wellness
education, psycho-education and
work readiness skills.

This service consist of mental health and substance
abuse rehabilitation services and supports necessary to
assist adults (ages 21 and older) in achieving
rehabilitative and recovery goals. Intended to reduce
psychiatric and addiction symptoms; and develop optimal
community living skills.
Interventions include the following but are not limited
to:

Behavioral Point System
Behavioral Contracting
Anger Management
Conflict Resolution
Psychodynamic in nature
Behavioral modifications
Independent living skills
Adaptive living skills
Our goal is to reduce
functional impairments associated with existing
emotional/
behavioral problems. We link consumers to various
community resources based on the consumer’s strengths &
needs assessment. We Foster growth and independence to
the individuals we serve by providing whatever level of
assistance is needed to assist consumers in meeting
their goals.

Intensive in Home therapy |
|
Intensive In-Home Services
Program is available for providing effective treatment for
children with serious emotional behavioral problems and
their families.
CAYFS intensive In Home
therapy services help sustain youth in their home setting
and eliminates the need for children to be placed in out of
home placement. CAYFS intensive In Home works with youth who
are at risk of being placed in out of home placement to
prevent removal from the home. The goals of this program are
to keep families together, to reduce risk of harm to
children and make families safe, and to assist families in
building community-base supports.
Therapy is intensive and is conducted in the
child's home by a team of professional’s counselors. The
counselor(s) may meet with the child and family in the home
several times a week for four to six months or longer. As
part of the process, the counselor may frequently work
closely with teachers, neighbors, extended family, even
members of the child's peer group. The counselor(s) is
available to the family 24 hours a day, seven days a week
when necessary.
IHT has been proven effective in helping
young people who might otherwise be placed in residential
treatment, detention centers, psychiatric hospitals or other
residential juvenile facilities. Youth who are returning to
their families from out-of-home placements can also benefit
from services.

nature & intervene to reduce the likelihood of a
recurrence.

Ensure linkage to needed community
services & resources Provide self help & living skills
training for youth

Provide parenting skills training to help
the family build skills for coping with youth's disorder

Monitor & manage the presenting
psychiatric & addiction symptoms

Work with caregivers in the
implementation of home based behavioral supports.
